I think I found the DRV fault problem when using foc and a solution for it, it would be nice if you can give it a try:

I think it is a good idea to discuss the features of the next iteration of the VESC with the community. I will start a thread about it when I start working on the next PCB, and it would be nice if everyone can contribute with what they would like to add based on their experience so far.

foc braking (using the 'full brake' in bldc tool) doesn't work for me properly either, but foc position control does hold a position very strongly.

I have the same or worse problem.
I had on my E-bike VESC set to FOC with currentcontrol and brake. It had given the error (blinking red 2x3 times) quite some times. Finally the vesc died.
I can access the Vesc with the bldc and see data and motor position and RPM etc.. but the motor does not start, instead it blinks red three times.

Benjamin, I could leave the VESC for you to have a look if you like to find out how it broke? -I will put it in my slot at CRF (it has a label 'VESC#1' written in a crocodile frame.

It seems one of the Mosfets got quite warm (indicated by dent the shrinkwrap).
Can I replace some components to fix it?
Which components do I need to order in that case?
